TWO late goals by substitute Callum England sealed a memorable victory to book Mathern's place in the semi-finals of the Centenary Cup.
England struck his first in the second 15-minute spell of extra time cutting in from the right wing before firing low into the far corner.
His second came as Underwood pushed for an equaliser, when he eluded a challenge from keeper Justin Bidgood and scored into an empty net.
England replaced injured striker Harry Ward just after half time and his introduction saw a change in Mathern's tactics, switching from playing through the middle to using the flanks of a pitch which resembled a ploughed field.
The match was switched to Underwood as a result of the still unresolved Gwent County League fixture mix up; a catalogue of errors in the original fixture list forcing lower leagues to revise their own fixtures as errors are amended month by month.
Mathern's first team were due to play New Inn in a County League game on the same day so their second team were ordered to switch this cup game to Underwood.
Mathern took the lead after Ward wriggled clear on the left and then cut the ball back from the goal line for Jack Stewart to score from 15 yards.
But a hotly-disputed penalty gave Underwood an equaliser, Josh Downing following up to score after his spot-kick was saved by Mathern keeper Oli Aleman – his fourth penalty save in as many weeks.
With Ryan Guscott and Harry Lewis outstanding in midfield, Mathern came back strongly and after Guscott was tripped, Wayne Duffield fired a 25-yard free-kick which left Bidgood stranded.
After the break, Tom Turner scored a second equaliser when Mathern were caught short at the back and despite the efforts of both sides, the score was 2-2 at the final whistle.
David Jackson's goal seemed to have won it for Underwood in the first half of extra time but Duffield stepped up to fire an unstoppable free kick a few minutes later to level it once again.
Then, with the prospect of a penalty shoot-out looming large, England took control and settled it with his two well-taken goals.
Jubilant Mathern boss Lee Baker said Duffield's second free-kick was "out of the top drawer", adding: "I don't think you will see a better free kick this season.
"Ryan (Guscott) was my man of the match and he and Harry (Lewis) were workhorses in midfield. It wasn't an easy game at all but I think we deserved it
"The team should be proud of themselves for the performance they gave. It was a very gutsy performance against a great Underwood side. We worked hard and battled as a team, but we didn't have it all our way.
"We had to earn our place in the next round."
