The Welsh Ambulance Service was under "too much pressure" on the night a Monmouth woman suffered the collapse which led to her death.
An inquest into the death of Jacqualine Davies heard that call centre staff were "too busy" to ask other ambulance services for help as she waited 42 minutes for a paramedic.
The inquest also revealed a shift at Monmouth ambulance station was not covered and, when a paramedic did arrive, his vehicle would not start.
Ms Davies' family have slammed the ambulance service after a draft report looking into the incident was "inadvertently disclosed" to Deputy Assistant Coroner Wayne Griffiths, delaying the inquest by more than two hours.
