Police are looking to talk to the driver of a black Ford Fiesta or Focus who drove off after a collision on the A40 in Monmouth last Saturday (15th February).
Officers were called to the dual carriageway just after midday after the black Ford, driven by a white male, believed to be between 20 to 30 years of age, was in collision with a green Vauxhall Agila driven by a man in his 50s, with a woman in the front passenger seat and two children, aged seven and 14, in the rear of the car.
As a result of the collision, the Vauxhall Agila left the carriageway and went down an embankment into trees on Chippenham.
The driver of the Ford did not stop and was last seen turning left at the traffic lights, into Monmouth.
The driver and children in the Vauxhall were taken to Neville Hall Hospital with minor injuries.
The woman was taken to the Heath Hospital in Cardiff by Air ambulance with serious leg injuries.
Anyone who may witnessed the collision or has any information about the driver of the black Ford is asked to call Gwent Police on 101 quoting log 300 15/02/14.
