Wok U Like, a takeaway at 88 Newport Road, Caldicot, Monmouthshire was given the score after assessment on July 22, the Food Standards Agency's website shows.
It means that of Monmouthshire's 61 takeaways with ratings, 34 have ratings of five and none have zero ratings.
